The Darrell Gwynn Foundation is currently seeking individuals in need of a new wheelchair. DGF is a non-profit organization who’s largest program is its wheelchair donation program. All of the wheelchairs donated are brand new and customized to fit the need of the individual. The following is the list of basic criteria that one must meet in order to qualify for the DGF program. If qualification is determined, then there will be a formal application which must be filled out.
– Applicant must be a permanent wheelchair user
– Applicant must be living with the injury or disability for at least six months
– Applicant must be between 2 and 45 years of age
– Applicant must be able to provide documentations from a licensed physician stating the permanency of the applicant’s disability
– Applicant, parent or legal guardian must demonstrate a financial need
– If applicant is covered by insurance, a letter from the insurance company stating the reason for denial must be submitted with the application
